You know that heavy, wound-up feeling after a stressful week? Your shoulders creep toward your ears. Your neck feels like concrete. Your sleep suffers. That feeling is not just in your head. It is your stress hormones doing their job — and sometimes overdoing it. The good news is that movement is one of the most powerful tools you have to bring those hormones back into balance. In this blog, I want to walk you through what the research actually shows about how movement changes your stress chemistry, why your muscles get tight and achy when you are stressed, and how a chiropractor can help you find the right kind of movement to feel better.

What Are Stress Hormones and Why Should You Care?

Think of your stress response like a fire alarm. When something threatens you, your brain automatically activates a system called the HPA axis. This releases cortisol, your main stress hormone. In small doses, cortisol is helpful. It gives you energy and sharpens your focus. But when the alarm keeps ringing because of daily stressors like work deadlines, poor sleep, or ongoing pain, your cortisol levels stay elevated for too long. That is when problems start. A 2023 meta-analysis by Moyers and Hagger in Biological Psychology confirmed this pattern. People who move regularly tend to have a steeper diurnal cortisol slope. That means their cortisol rises properly in the morning and drops at night, exactly the healthy rhythm you want. People who are sedentary and stressed often lose that rhythm entirely.

Why Your Muscles Feel Tight and Your Pain Gets Worse When You Are Stressed

When cortisol stays high, your body holds onto tension. Your muscles tighten like a clenched fist that cannot let go. You might notice that dull pain in your shoulders or that achy pain across your lower back. This is not your imagination. Your stress hormones are literally changing how your muscles behave. They stay “switched on,” waiting for a threat that never arrives. Over time, that tightness becomes your new normal, and it can lead to injuries because tight, fatigued muscles do not absorb force well.

The science backs this up. A 2023 review in the Journal of Clinical Medicine mapped the biology of stress intolerance in chronic pain populations. The authors found that when cortisol becomes dysregulated, it fails to properly control inflammation. This creates a vicious feedback loop: your pain increases your stress, and your stress increases your pain. A 2026 systematic review by Vyverman and colleagues confirmed that higher cortisol levels and reduced heart rate variability are associated with greater pain sensitivity. Your stress system and your pain system are not separate,  they are deeply connected.

How Movement Resets Your Stress Hormones

When you move your body, you send a powerful signal to your brain. That signal says: “We are okay. You can turn the alarm down now.” Over time, regular movement teaches your stress system to recover faster and overreact less. I like to think of it as training a puppy. At first, the puppy barks at everything. But with consistent, patient training, the puppy learns what is a real threat and what is just the postman.

A landmark 2025 network meta-analysis by Li, Huang, and Zhu in Sports pooled 44 randomised controlled trials with over 3,200 participants. They found that exercise produced moderate reductions in cortisol among people with psychological distress. Importantly, they identified a dose-response relationship. The optimal dose was around 500 MET-minutes per week, roughly 150 minutes of moderate activity like brisk walking five days a week. Yoga ranked highest for cortisol reduction, followed by qigong and multicomponent exercise. Here is the hidden pitfall: high-intensity interval training actually tended to increase cortisol. If you are already highly stressed, jumping into intense workouts can make your cortisol problem worse, not better.

Movement, Proprioception, and Why Your Brain Needs to Know Where Your Body Is

There is a less obvious way that movement helps your stress. It has to do with proprioception — your body’s sense of where it is in space. Close your eyes and touch your nose. That is proprioception at work. When you are stressed, this sense gets fuzzy. Your brain becomes less accurate at tracking your body. You might feel clumsy or “off.” Your injuries become more likely. And poor proprioception keeps your nervous system on high alert, which keeps your stress hormones elevated.

Research by Haavik and colleagues has shown that spinal joint dysfunction alters proprioceptive input from the deep paraspinal muscles. In a 2021 review in the European Journal of Applied Physiology, they outlined a model showing how this altered input changes the way your brain processes movement information. A 2025 randomised controlled trial in Scientific Reports took this further, studying 142 adults with chronic low back pain. The researchers found that spinal manipulation influenced proprioceptive weighting, how your brain prioritises balance information from different body regions. Controlled, deliberate movement and chiropractic care can sharpen your proprioception. This tells your brain: “I know exactly where I am and I am safe.” That is a crucial and often overlooked part of the stress-movement puzzle.

The Right Exercise Dose: Why More Is Not Always Better for Stress Hormones

You might think harder exercise is always better. But when it comes to your stress hormones, that is not the case. If your body is already running on stress, pushing too hard adds fuel to the fire. The sweet spot is moderate movement, a walk, a swim, a gentle strength session, or yoga. These activities calm your nervous system without overwhelming it.

The 2024 BMJ network meta-analysis by Noetel and colleagues, one of the largest ever conducted on exercise and mental health, analysed 218 randomised controlled trials. Walking, jogging, yoga, and strength training all showed meaningful improvements in depressive symptoms. Critically, prescribed exercise programs with specific parameters were more effective than programs where people chose their own intensity. This reinforces a key point: exercise dosage matters. Too little may not shift your cortisol patterns. Too much can worsen HPA axis dysregulation. This is exactly why guidance from a health professional who understands exercise prescription is so valuable for your recovery.

How a Multimodal Chiropractic Approach Helps Your Stress, Pain, and Movement

At our clinic, we use a combination of three things: soft tissue therapy, chiropractic adjustments, and exercise prescription. Each one targets a different part of the puzzle. Soft tissue therapy helps release the muscle tightness that stress creates. It works on those sore, aching muscles directly by activating pressure receptors and increasing vagal nerve activity, which can temporarily reduce cortisol and ease your pain. Chiropractic adjustments help restore proper movement to your joints. When your joints move well, your proprioception improves and your brain gets better information about your body. Prescribed exercise then retrains your stress system over time.

A 2025 pragmatic randomised controlled trial by Amjad, Niazi, Haavik, and colleagues in PLOS ONE provides some of the strongest evidence to date. They assigned 106 adults with subclinical spinal pain to either 12 weeks of chiropractic care or sham care. The chiropractic group showed significant increases in brain-derived neurotrophic factor (BDNF), a key marker of neuroplasticity. At the 16-week follow-up, the sham group showed increased blood cortisol, while the chiropractic group showed a trend towards reduced long-term cortisol exposure. These are early but promising findings. When a chiropractor prescribes exercise alongside hands-on care, they can tailor the dose to your current stress level. A person dealing with chronic pain and high stress needs a very different exercise prescription than someone in peak condition. Getting that dose right is what makes the difference between exercise that heals and exercise that overwhelms your system.

Summary

Your stress hormones are not your enemy. They are a survival system that can get stuck in overdrive. When cortisol stays elevated, your muscles tighten, your pain increases, and your proprioception suffers. Movement is one of the most evidence-backed ways to bring that system back into balance, but the type and dose of movement matter enormously. A multimodal chiropractic approach that combines soft tissue therapy, adjustments, and tailored exercise gives you the best chance of addressing your stress, pain, and tightness as the connected system they truly are. You do not have to push through pain or punish your body with intense workouts. Sometimes, the most powerful thing you can do for your stress hormones is move gently, move consistently, and move with guidance.
